  • Cigna Open Access Plan
  • Coverage is effective the first day of the month following date of hire
  • No cost share, such as deductibles, co-payments or co-insurance for in-network preventive services (ie, well-baby visits, well-woman checkups, annual physicals)

Dental

  • Cigna Dental
  • 100% coverage for preventive and basic services
  • 50% coverage for major services
  • Coverage is effective the first day of the month following month of hire

Vision Insurance

  • Cigna Vision
  • $10 Co-payment
  • At participating providers, members pay a $10 office visit co-pay plus a $25 hardware co-pay and charges over and above the negotiated amounts for frames and lenses. Plan covers exam and lenses every 12 months and frames every 24 months
  • Coverage is effective the first day of the month following month of hire

Life Insurance

  • Accidental Death and Dismemberment
  • Long Term Disability and Short Term Disability
  • Cigna Life Insurance Company
  • Life/AD&D based on 2x annual salary up to $500K without evidence of insurability
  • LTD provides 60% of monthly earnings after the first 90 days of disability
  • STD provides 60% of weekly earning beginning of the 1st day of disability for injuries and on the 8th day of disability for illness
  • Voluntary supplemental life insurance available for you, your spouse, domestic partner and/or your dependents

401(k)

  • Fidelity
  • Can defer up to 60% of regular earnings
  • Salix matches 100% of the first 6% of deferred salary
  • Eligible to enroll immediately upon hire
  • Eligible for company match the first day of the first quarter following date of hire: January 1, April 1, July 1, October 1

Direct Deposit

  • Semi-monthly pay on the 15th and last day of each month

529 College Savings Plan
